Pre-use check for any hired-in equipment — Achilles UVDB B2.14.1.

AMWS — Hired Equipment Suitability Check (FORM_HIRE_CHK)

Complete once per item of hired equipment, on first delivery to AMWS or on first day of use, before the kit goes into service. Achilles UVDB B2.14.1 requires this check.

If anything ticks ‘No’, the kit doesn’t go to work — quarantine and call the hire company.

1. The kit

Off the plate on the kit itself, not the hire note.

2. On delivery — checks

Statutory inspection in date (LOLER / PUWER / electrical)(Required)
Check the cert on the kit itself or in the cab — should be inside the last 6 / 12 months as applicable. Photo of the cert is good evidence.
Visual inspection — no obvious damage(Required)
Walk around. Glass, hoses, tracks, cables, paint, oil leaks. Anything that looks wrong, it probably is.
Operator competence verified(Required)
Whoever’s going to use this kit holds the relevant ticket / card (CPCS, NPORS, IPAF, PASMA, etc.) and it’s still in date.
RAMS in place for this kit's use(Required)
Method statement covers what we’re doing with this kit. If we’re doing something the RAMS doesn’t cover, raise it before going to work.
Refuelling / oils / consumables plan in place(Required)
Spill kit on the truck, refuelling location/method known, drip tray available where relevant.
Manufacturer's manual / instructions on the kit(Required)
Inside the cab, on the kit itself, or with the hire paperwork.
Any reading required to make sense of the above — defects found, photos taken, hire-company contact made, when the kit can be used.

    Filing: the signed check + photos are kept against the kit’s entry in APP_14 Calibration / Equipment Register until off-hire.
