AMWS — Accident & Incident Investigation Report (HS_FM_02)

Use this form to record an accident (someone or something was harmed) or an incident (something went wrong but no harm — but more than a near miss).

For something unsafe spotted but no incident, use the Near Miss / Hazard Report instead. For pollution, use the Spill Response Form.

This form has five short sections. Skip injured-person and witness sections if they don’t apply. Aim for 5-10 minutes — get the facts down while they’re fresh.

2. Why did it happen? (immediate cause)

The direct trigger — the strap that slipped, the cone that got knocked, the cable that wasn’t traced.

3. Why did it happen? (underlying cause)

Behind the immediate cause — was there a RAMS gap? Equipment issue? Briefing not done? Time pressure?

What happens after you submit

The HSQE Consultant gets the full report immediately, with photos attached. Major / Lost-time / Fatality incidents escalate automatically to Aaron Mason. If a RIDDOR notification is needed, Sean will complete F2508 within the statutory window. You’ll receive a copy of the report by email and a close-out note when remedial actions are signed off.
