AMWS — Spill Response Form (HS_FM_30)

Use this form when any spill or release happens on an AMWS job or at the Northampton HQ — fuel, oil, hydraulic fluid, chlorination chemicals, drilling muds, slurry, or any other substance that ends up where it shouldn’t.

Make it safe first, fill in the form second. If the spill has reached a drain, watercourse, ditch, or land outside the works area, you MUST also call Aaron Mason immediately on 07XXX (placeholder).

A watercourse — river, stream, ditch, pond, lake

Unsure — please check on site and update if needed

This decides who gets notified. Be honest — over-reporting is far better than under-reporting.

Containment actions taken

Source of spill stopped (valve closed / drum righted / equipment shut off)

Spill kit deployed — granules / absorbent pads

Drain seal / blocker fitted to nearest drain

Sand / earth bund built around spill

Contaminated materials bagged and labelled for disposal

Site visually clean — no further action needed
